    Educational Requirements: Master's or Doctorate (MS/DPT) in Physical Therapy.

  • Licensure: Must hold a valid, state of New York Physical Therapy License in good standing.

  • Experience: While new graduates are warmly welcomed, previous experience in a Skilled Nursing Facility setting will be considered an asset.

  • Skills and Attributes: Demonstrated ability to deliver compassionate, effective care, strong interdisciplinary team collaboration, and excellent communication skills.

    Responsibilities:

  • Deliver high-quality physical therapy services aimed at enhancing the health and well-being of our residents.

  • Administer physical therapy treatments as prescribed, ensuring seamless communication with referring physicians.

  • Write, update, and maintain accurate evaluations, progress notes, and discharge summaries for residents in the physical therapy program.

  • Collaborate with an interdisciplinary team to create and implement customized care plans to improve or maintain residents' functionality.

  • Provide essential input in Resident Care Planning meetings and represent the Physical Therapy department as needed.

  • Maintain accurate statistical records of residents' treatment progress and outcomes.
