Clinical Supervisor - Home Health/Hospice Services

Responsibilities:

  • Leadership and Guidance: Promote our organizational mission, vision, and values. Provide expert guidance and support to staff, ensuring the delivery of home health/hospice services meets regulatory, quality, and practice standards.

  • Operational Management: Oversee day-to-day operations, aiding with staff scheduling. Monitor and review clinician activity reports, identifying and addressing improvement areas.

  • Patient and Family-Centered Care: Guide clinicians in maintaining safe and centered care tailored to patient-family needs.

  • Interdisciplinary Coordination: Lead case conference/IDT meetings to ensure cohesive coordination of care and effective interdisciplinary group involvement.

  • Policy and Performance Management: Assist in developing, revising, and evaluating organizational policies and procedures. Evaluate team member performance, providing necessary coaching and disciplinary actions in line with Human Resources policies.

  • Strategic Participation: Actively participate in leadership and committee meetings, contributing to the department's overall management.

  • General Duties: Perform other related responsibilities as necessitated by the role.

Qualifications:

  • Education: Associate's degree preferred.

  • Licensure: Must hold a current Registered Nurse license.

  • Experience: At least 2 years of hospice experience and 1 year in a supervisory role are required. A background in home health, general medical surgical, or patient population-specific nursing (i.e., geriatrics, pediatric, etc.) is advantageous.

  • Availability: Willingness to serve on-call as needed.

  • Transport: Must possess reliable transportation and a valid driver's license.
